What Is A Physician Assistant?
Physician Assistants are medical professionals who diagnose illness, develop and manage treatment plans, prescribe medications, and often serve as a patient’s principal healthcare provider. PAs are committed to team practice with physicians. PAs are trained to practice medicine in graduate-level programs using a curriculum modeled on medical school education.

How Do I Schedule An Appointment For A First Visit?
Simply call our office or send us an e-mail. We will be happy to schedule an appointment for you. When you call to schedule your appointment, our front office staff will request some basic information from you. We also offer same-day appointments.

What Information Do I Need To Bring To My First Visit?
Please arrive 15 minutes early to check in. To assist us, please bring your insurance card, method of payment and a photo ID. We ask you bring a list of any medications, vitamins and/or herbal supplements you are currently taking. If you are bringing your child, please bring his/her immunization records as well.

We are in network with the following major insurance carriers:
Aetna, Blue Cross Blue Shield, Cigna, Medcost, Medicaid and Medicare

What Payment Options Are Accepted?
We are committed to providing medical care that is affordable for every family. Payment or insurance co-pays are due when services are rendered.
A member of our staff can review our payment options with you to find one that best accommodates your financial needs. We accept Visa, MasterCard and Discover.

What If I Don’t Have Health Insurance?
If you do not have insurance, we have a self-pay membership option available. For more information, please email us at info@tandemprimary.com.

What Can I Expect During My First Visit?
After you complete the necessary registration forms, we will take some time to get you acquainted with our office. We will discuss our financial policy with you and help set up a financial plan that is best for you. We are always available to answer any questions you may have concerning your account and/or treatment.

How Can I Get A Prescription Refill?
Please call your pharmacy to send us a refill request. Once you are an established patient, the preferred method is to send your request via the patient portal.
We ask that you allow 3 business days for completion. Please keep in mind that some prescriptions may require a follow-up exam to determine the need of a refill.

Why Do I Need A Referral To See A Specialist?
Most often, insurance companies/plans require a referral to a specialist in order for them to cover their portion of the services provided. Be sure to contact your insurance provider to determine if a referral is needed to see a particular specialist.

How Can I Get My Medical Records or Send Them To Another Office?
In order for us to transfer your medical records to another office, we need to have a Medical Records Release form signed and dated by you. This allows you to specify to whom we are able to release your medical information and for us to protect your medical information under the HIPAA regulations. Please allow 5 days to process your request. Please contact us for additional information.
